| | COVID-19 Mandated Daily Screening
Per guidance from the Governor's Office regarding Phase 4 - re-openings, mandatory daily health screenings are required to be completed DAILY by any employee, student, vendor or visitor entering a business or school facility. Please complete the following survey. If your response is "yes" to ANY of the below questions, DO NOT ENTER THE BUILDING until further notice, answers will be reviewed by the school nurse and district administration daily in order to ensure full compliance with the Governor's regulations.

| | | | | | | | Have you traveled internationally or to any state which does not border New York State (NYS) in the past 10 days?
For travelers who were in another state for more than 24 hours:
- Travelers must obtain a test within three days of departure from that state.
- The traveler must, upon arrival in New York, quarantine for three days.On day 4 of their quarantine, the traveler must obtain another COVID test. If both tests comes back negative, the traveler may exit quarantine early upon receipt of the second negative diagnostic test.
For travelers who were in another state for less than 24 hours:
- The traveler does not need a test prior to their departure from the other state, and does not need to quarantine upon arrival in New York State.
- However, the traveler must fill out our traveler form upon entry into New York State, and take a COVID diagnostic test 4 days after their arrival in New York

Do you have a temperature of 100.0 F° or greater, chills, cough, sore throat, shortness of breath or difficulty breathing, new loss of taste or smell, headache, fatigue, nausea or vomiting, diarrhea, congestion or runny nose, and/or muscle or body aches? If you have any of these symptoms, do not enter the building. Based on your responses you are not eligible to enter a district building and is required to:
- Remain home and self-quarantine for a period of 10 days.
- Contact your county's department of health office and report your incodent.
It is also strongly recommended that you contact a healthcare professional if you have
not done so already.
