COVID-19 Mandated Daily Screening

Per guidance from the Governor's Office regarding Phase 4 - re-openings, mandatory daily health screenings are required to be completed DAILY by any employee, student, vendor or visitor entering a business or school facility. Please complete the following survey. If your response is "yes" to ANY of the below questions, DO NOT ENTER THE BUILDING until further notice, answers will be reviewed by the school nurse and district administration daily in order to ensure full compliance with the Governor's regulations.

In accordance with the NYS Travel Advisory, have you traveled internationally in the last 10 days?

If yes, after arrival in the U.S. travelers must either quarantine for 7 days with a test 3-5 days after travel, or quarantine for the full 10 days without a test. This requirement applies to all international travelers whether they were tested before boarding, are recovered from a previous COVID infection, or are fully vaccinated.

Do you have a temperature of 100.0 F° or greater, chills, cough, sore throat, shortness of breath or difficulty breathing, new loss of taste or smell, headache, fatigue, nausea or vomiting, diarrhea, congestion or runny nose, and/or muscle or body aches? If you have any of these symptoms, do not enter the building. Thermometers are available at the main offices.
