COVID-19 Mandated Daily Screening
Per guidance from the Governor's Office regarding Phase 4 - re-openings, mandatory daily health screenings are required to be completed DAILY by all students (in-person and virtual). Please complete the following survey. If your response is "yes" to ANY of the below questions, DO NOT ENTER THE BUILDING until further notice, answers will be reviewed by the school nurse and district administration daily in order to ensure full compliance with the Governor's regulations.

In the past 10 days, have you been in close contact (within 6 feet for at least 10 minutes) with anyone who has been diagnosed with a COVID-19 virus infection or tested positive for a COVID-19 virus infection thru a swab test (not a blood test)? |

In the past 15 days, have you traveled outside of NYS other than to Massachusetts, Pennsylvania, Vermont, New Jersey or Connecticut?

Information for Travelers:
- For travelers who were in another state (except those listed above) for less than 24 hours:
- You do not need to quarantine, but on return to NYS, you are required to complete travel paperwork and take a COVID diagnostic test 4 days after you return to NYS.
- For travelers who were in another US state other than above, for more than 24 hours, travelers can either do paperwork upon their return to NYS OR take a series of tests (as below) to end the quarantine early.
Travelers must :
- Obtain a test WHILE IN THE OTHER STATE, within 72 hour of their arrival back in NYS AND
- The traveler must complete paperwork upon arrival in NYS and begin quarantine in their home AND
- The traveler must take ANOTHER test on day 4 of being back in NYS
- IF both tests are NEGATIVE, the traveler may end their quarantine.
- For travelers that travel OUTSIDE OF THE UNITED STATES, you will need to complete paperwork upon your return to NYS and quarantine in your home for 10 days.

- Remain home and self-quarantine for a period of 10 days.
